If you look very carefully, you will see a tiny basal cell carcinoma (BCC) in the centre of the rectangle.  Basal cell carcinoma is the commonest of all cancers, and the face is the commonest site.  When diagnosed this early, a BCC is easily removed with little if any cosmetic defect.
